Registration Process
How to Request Accommodations
To receive accommodations at FSC, you will complete a confidential registration process with Student Disability Services (SDS), and we will guide you through each step.
Accommodations apply from the date they are approved forward, not retroactively, so the earlier you start, the sooner your support is in place. We recommend reaching out before classes begin.
The Key Is to Start Early
Starting early matters because some reviews take time and, for housing, placement depends on what is available when your eligibility is confirmed. To improve your chances, aim for the priority deadlines below, and read on for what to submit.
- If you are newly admitted, begin as soon as you receive your admission notice.
- If you are already enrolled, reach out as soon as a need for accommodation comes up.

Your healthcare provider will need to complete an FSC documentation form that establishes your disability or temporary health condition. Your documentation supports your request, but it does not determine your accommodations. Our staff reviews it and decides which accommodations are appropriate and reasonable to remove barriers to access. Accommodations provide equal access to courses, housing, and campus programs. They are not designed to guarantee a particular outcome.
Submit a separate form for each type of accommodation you are requesting, using the links below.
After you submit, you will receive an email confirming we have your request and documents.
Academic Accommodations
For accommodations related to your coursework.
Medical Single
For a medical single room in campus housing. Based on availability of spaces.
Emotional Support Animal
For an emotional support animal in campus housing. ESA approvals are reviewed each year, and we may ask for updated documentation.
Other Accommodations
For needs such as dietary accommodations, low-use restroom access, or elevator access.
- 2
Once we have your request and documentation, we will review it and follow up with you for one of two reasons: to ask for any missing documentation, or to set up a time to discuss the accommodations we are proposing.
From there, you and SDS work together, through an interactive process, to determine reasonable and appropriate accommodations, and whether you are eligible.
Priority Deadlines
You can request accommodations at any time, and we will always consider your request. These dates are priority deadlines, not cutoffs: meeting them gives you the best chance of having your accommodations, especially housing, in place when you need them. If you are admitted after the deadline, submit your request as soon as possible to begin the accommodation process.

What Happens Next
Here is what to expect once your review is underway:
We will let you know in writing, within a reasonable time, what else we need and why, so we can accurately review your request.
We will work with you to identify appropriate and reasonable accommodations. We will notify the appropriate parties, including but not limited to, instructors, Residence Life, and Campus Safety, of your appropriate accommodations.
We will confirm your accommodations in writing and share your Accommodation Plan with the offices that need it, such as your instructors, Residence Life, and Campus Safety. Plans are emailed a week before the semester starts or within two business days of approval, and you will be copied so you know your instructors have received it.
We will explain the reasons in writing, within a reasonable time, and include instructions on how to appeal.
