Use this form to report any possible violations of Florida Southern College's Non-Discrimination and Harassment Policy such as discrimination based on race, creed, color, national origin, gender, gender identity, sex, sexual orientation, marital status, pregnancy, religion, age, disability, political opinions or affiliations, genetic information, veteran or military status, and any other protected category under applicable local, state, or federal law.
