Directions

If you experience or witness an incident of bias, discrimination or harassment, please report it. The formal complaint form allows Florida Southern College to investigate claims of discrimination. Please note, per Title IX rule, complaints of discrimination based on gender must be formally filed (not anonymous) to be investigated.

Anonymous complaints of discrimination are welcome though they may make an investigation more difficult to pursue. Please choose the anonymous reporting option that applies to your situation.
