Immunizations
Florida Southern requires that all students have the following immunizations.
Required Immunizations
- MMR/Measles, Mumps, Rubella Vaccine
- Hepatitis B Vaccine
- Meningococcal Meningitis/MCV4 (Menactra/Menveo) Vaccine
Recommended Immunizations
- Td (Tetanus/Diptheria or/and Tdap (Tetanus/Diptheria/Pertussis)
- Varicella (Chickenpox)
- Hepatitis A, HPV, Polio
- Meningitis B
Immunization Verification & Compliance
Florida Southern College partners with Med+Proctor for student immunization compliance and verification. All students, regardless of age, must submit immunization records to be reviewed by Med+Proctor. Late, incomplete, or inaccurate information will prevent move in to residence halls and/or class attendance.
Steps:
- Go to www.medproctor.com to create an account using your personal email as your ID.
- Complete the required workflow steps including confirming your email address.
- You will need to upload proof of immunization. FSC will accept the Med+Proctor Immunization Certificate, the FSC Immunization Health History Form, the official State of Florida Immunization Form, local health department forms, stamped high school transcripts, military immunization forms, and physician’s office records.
- Have your healthcare provider complete the certificate/form.
- Upload the completed certificate/form to your account.

It is free to submit your forms. To do so, select the gray button.
To have Med+Proctor access state immunization forms directly (so that you won’t need to submit any forms) and/or to enroll in lifetime access to Med + Proctor, select the green button and pay a one-time fee of $10.
Vaccine Waiver Requirement:
Vaccines may only be waived in the event of submitting a signed Religious or Medical Exemption Release Form. Students under the age of 18 who are waiving the required immunization must have a parent or guardian sign.
